SUPPORTING TRANSITION

We recognise that transition can be difficult, particularly for a child with SEND and we take steps to ensure that any transition is as smooth as possible.

 

On first entering our school:

 

*we will meet with you as parents to discuss your child’s needs in full

*we will liaise with any previous setting where possible to share information to help us prepare for your child arriving at Pentland

*we will provide a gradual transition programme if needed

*we will contact any other professionals involved in supporting the care of your child to take advice

 

If your child is moving into another school or moving into Year 3:

 

*we will contact the school SENCO and ensure that they know about any special arrangements or support that needs to be made prior to your child’s arrival

*we will make sure that all your child’s records are passed on quickly to the receiving school

*wherever possible we will arrange additional transition visits

 

When moving classes in our school:

 

*transition meetings are held between current and receiving class teachers to share all  information and assessment data
